By making Health Super your only super fund, you'll know your super is being looked after by the fund that cares about you and your needs.

It's easy - follow these steps.

Ensure that from now on your employer's super contributions are made to Health Super

 

Step 1 - Complete the Fund nomination form

Step 2 - Hand the completed form and the letter of compliance (on the back of the form) to your employer - don't return the form to Health Super

Roll all your other super funds into Health Super

 

By having more than one super fund you're paying more than one set of fees. That's a waste of your hard earned super.

 

Step 1 - Complete the Consolidate your super form

Step 2 - Certify your ID: a certified copy of proof of your identity is needed to process your rollover. Details of how to do this are located on the back of the form.

Step 3 - Send the completed form and certified copy of your ID to us at:
