When you join Health Super, you will automatically receive Death, Total and Permanent Disablement and Income Protection cover, subject to you being eligible.
You may already have existing insurance cover above Health Super's automatic level with another provider. If you are considering consolidating your super with Health Super, you also have the option to consolidate your insurance cover, all at the same time. To make it easier for you, Health Super will match your existing level of cover when you consolidate your insurance with us, without the need to provide health evidence, subject to our terms and conditions.
What to do next
Follow these easy steps to transfer your existing cover to Health Super. To apply, simply:
- Complete all sections of the Insurance transfer application and declaration form and provide the required details
- Acknowledge the Duty of Disclosure section of this form; and
- Sign and return the completed Insurance transfer application and declaration form to Health Super Pty Ltd, Locked Bag 2900, Collins Street West VIC 8007.
