find the details of your other super accounts (e.g. old statements)

collect the number of forms you need
Just download the standard ATO Request to transfer whole balance of superannuation benefits between funds, and print out as many copies as you need. You will need to use a separate form for each of your additional super accounts.

complete the forms with all your details
Make sure you supply the original copy of each Request to transfer whole balance of superannuation benefits between funds with all the information requested, otherwise your application may be delayed.
