Choosing Health Super as your default fund
Make the right choice when you select your default fund for employees.
There are two simple steps you can implement to select Health Super as your default fund.
Selecting Health Super as your default fund
All you need to do to make Health Super your employer (or default) fund is to put Health Super's details in Part A of the Standard Choice form. To do this:
- Download a Word version of the ATO's Standard Choice form
- Type in the information in blue below at Question 2 in Part A at the top of the form:

- Answer the other questions in Part A of the form.
- Save, print and hand out as many copies as you need.
